annual leave

issue one: I put in a sick leave for almost a month ago for an appt..

I’m just now finding out it was denied even though she put it in denied in the computer I was not notified..

it’s been a month, was never given a slip back

issue 2:  I put in two other request of annual say and she has denied all of my request.

but she’s approving other others. What should I do?
 
Please make sure that each and every time you submit a leave request that you have the supervisor sign and date the area directly ABOVE the black line. This action does not approve or deny the leave but acknowledges the submission. At least you would have your copy with this on the slip. I'd have to do a bit of looking but I believe it was @Dominator who had a thread regarding the topic of "proof that slip was / was not returned timely or even returned at all.

The most reliable evidence is placing leave through eLRA through LiteBlue and taking screenshots of each screen as you work your way through and finally get your confirmation number. eLRA's location is found in 3 simple steps.

I haven't personally had any integrity challenges to eLRA requests, but I grasp that some offices, on occasion, have obstinate management teams and/or union representatives not yet experienced with overturning inappropriately denied leave.

The second (less defensible) method is through a 3971, Request of or Notification of Absence.

The key element, as @neciat pointed out, is management's SIGNATURE AND DATE in the box indicated below.

Once management signs and dates it, don't just walk away without your evidence. Take a photo of it with your phone for quick reference, then:
  1. Take your original slip to the copy machine and make 2 copies.
  2. Staple the original to 1 copy and give it to management.
  3. Keep the other copy for your records.
