As the most senior rca should you be awarded a hold down for 90 days due to a regular being sick or illness. The hold down was given to the newest hired rca who is the primary sub on the route. Mgmt and union says it is only like asking for vacation for 3 months or getting a sick leave for 3 months or the regular has a definite time of coming back to work (90 days).  The only situation that I can do a hold down is if the reg need more extension after the initial 90days leave request and indefinitely don’t know when he can come to work?

The article you referenced refers to a vacancy in the leave replacement ranks, not the absence of the regular.

As stillfirstclass has already pointed out when any regular is going to be out for any period of time the primary RCA assigned to the route gets the hours.

When regular is absent for 1 day or 720 days, article 30.2.D applies.......... aka Matrix

If a sub is primary on the route, even if it's their first day on route and regular goes on LWOP for union business for next 15 years, that sub gets those hours.

Sounds like total luck of the draw! Now, if there was NO Primary assigned to a route and it is known that the Regular will be out or off the route for more than 30 days, mgmt can bid the Primary position out. If nobody wants the primary (it happens), mgmt can hire a sub for the "hold down" and/or the 2nd/3rd would run it when not on their Primary.
