I called HR Shared Services to find out about how to start the FMLA process. They informed me to submit a 3971 to management & that would get the process in motion and I would then be given a case number. I never got one, so I'm assuming he never submitted my 3971. My surgeon released me back to work (maternity leave), but they left the restrictions part blank, because I didn't have any. I called him the afternoon after the appointment & he said that I needed to submit Dr's note to postal nurse. I did that and she emailed me back saying she couldn't clear me without a medical conditions form. I wish they would have told me that, that way I would have had it completed when I was in the office. Why wasn't I ever informed that this form was needed? When I went on maternity leave the last time the Dr's note was sufficient and he put me on the schedule right away. I had no idea I needed anything else. He doesn't care for me much and knew that I didn't have enough leave left to cover me for these extra days. Who's job was it to inform me, the manager or HR, that this form was required to come back? I was released on a Saturday but according to him I didn't get cleared until Thursday afternoon. I asked the nurse to send me confirmation as to when I was released and she did not. I wanted it because I don't trust my manager and he does "petty" things such as this when he sees a loop hole.
Should the process to be cleared to return to work take 5 entire days?
